If you are coming to your appointment for the first time (Initial visit), please make sure to fill out the Intake forms, together with the Wellness pre-screening forms. Those forms can be found in your appointment email confirmation and text reminders.
Please arrive on time. You will be held responsible for your time lost and your appointment time will not be extended and the full amount will be charged regardless.
We are trying to minimize the use of papers, so we encourage sending email receipts instead of printing it. But if you still prefer your receipts to be printed please let us know at the time of your appointment.
